Do you have a scheduled hospitalization? For your stay to go smoothly, it is important to be well prepared.
To do this, think of your admission file in order to benefit from adequate care:
- Have your identity documents and your social security coverage on hand. If you are covered by CNAM or another health insurance, bring the corresponding documents with you as they will be requested of you at the admissions office. The administrative team is fully available to provide any assistance you may need.
Documents to bring
- On the day of your admission, take the following documents :
- Your identity card or passport
- Your Social Insurance, Mutual or Private Insurance Card
- Your medical documents: radiological images, medical reports and tests etc.
- If you are following a treatment, your prescription or the list of medications you are taking and their dosage
